1. Please include in NOTES section if you would like your credit card charged just once, automatically every four weeks, or automatically every quarter.
2. We will deduct 3% Family and/or 3% Quarterly (12-week) Discount if applicable.
3. You will be notified by e-mail within 2 weeks confirming registration.

How to Register WITH OR WITHOUT PAYMENT by MAIL:

1. Print the registration form, complete the form, and mail to: Johansen Olympia Dance Center, 412 Jefferson St. SE, Olympia, WA 98501.
2. NO TUITION required until classes begin OR you may pay by cash, check, Visa, MasterCard, or Discover (non-refundable.) WE TAKE AN UNPAID PHONED REGISTRATION AS A FIRM COMMITMENT, so please let us know if you change your mind. If you pay, deduct 3% Family and/or 3% Quarterly (12-week) Discount if applicable.
3. To register an additional student from same family, please include her/his name, birthdate, class/classes, day/days, time/times on a separate registration form.
4. You will be notified by e-mail within 2 weeks confirming registration.

How to Register WITH OR WITHOUT PAYMENT by PHONE:

1. Please call 360.943.8011 anytime (when we're not at the studio we answer the phone from our home office.)
2. Provide information on the registration form.
3. NO TUITION required until classes begin OR you may pay by cash, check, Visa, MasterCard, or Discover (non-refundable.) WE TAKE AN UNPAID PHONED REGISTRATION AS A FIRM COMMITMENT, so please let us know if you change your mind. If you pay, deduct 3% Family and/or 3% Quarterly (12-week) Discount if applicable.
